The credit certificate uploaded is genuine, unaltered, and was lawfully issued to me as a result of my participation and finish in the tournament described above.
I am the sole rightful owner of this credit and have not previously transferred, sold, assigned, or submitted it to any other party or platform.
The credit has not been partially or fully redeemed at the issuing pro shop or elsewhere prior to this submission.
I authorize Caddie Creds to contact the issuing pro shop to verify the credit amount, authenticity, and remaining balance.
I understand that Caddie Creds points are non-transferrable and non-refundable once issued, and are subject to the Caddie Creds Terms of Service.
I acknowledge that submitting false, fraudulent, or previously redeemed credits may result in account suspension and potential legal action.
Pro members receive full 1:1 point conversion. Free plan members receive 60% of the credit value as points, as outlined in the current plan terms.

This authorization is valid for this submission only and does not grant Caddie Creds any ongoing rights to access or redeem credits on my behalf beyond the amount stated in this form.
